INDOOR ADMISSION:
- Kindly contact the reception counter for admission form.
- Fill the admission form in capital letters.
- Provide your reachable contact details to the reception.
- Please select the class of bed at the time of admission.

- Patient admitted for planned surgeries, additional; 100% of total surgery package amount should be paid as deposit along with the regular above mentioned deposits.
- Deposit amount will be adjusted / refunded, as per the actual bill at the time of discharge.
- Kindly ask for Hospital Tariff Card / Paper at the reception for further details or speak to the Hospital billing department.
MODE OF PAYMENT:

- DEMAND DRAFT
- CREDIT / DEBIT CARD
- NEFT / RTGS / IMPS TRANSFER
Note: D.D. should be in favor of “CENTURY RAYON HOSPITAL TRUST” & payable at MUMBAI
MEDICLAIM RELATED QUERIES:
- All Mediclaim related queries shall be answered between 12 noon – 5 pm on working days by the management team.
- Kindly get the Re – reimbursement form of your concerned TPA / Insurance Company prior to discharge to avoid delay.
VISITING HOURS:
- Following are the visiting hours for ICU patients:
- MORNING – 11am – 12 noon
- EVENING – 05 pm – 07 pm
- Children’s below 15 years are not permitted inside the I.C.C.U. / I.C.U.
- Do not overcrowd the wards with multiple visitors.
PHARMACY
- Hospital has an in – house pharmacy, the payment of it would be part of your final bill.
- As these medicines are not charged at MRP hence breakup of these are not provided at the time of discharge, same can be obtained by putting a request to the billing executive in the format provided.
GENERAL RULES:
- Do not smoke / spit / chew tobacco or gutka inside hospital premises.
- Do not sit on patient’s bed.
- Please put domestic waste in the dustbins provided with black bags, do not throw general wastes in red / yellow bags & also do not litter in the hospital premises.
- Kindly keep your mobile on vibrator mode and follow the mobile etiquettes.
- Please take care of your belongings. Hospital Management shall not be responsible for any thefts / damage of your personal belongings or valuables.
- Strictly do not keep mobile phones / cash / jewellery etc. along with patients.

YOUR RESPONSIBILITIES
AS YOUR CARETAKER, WE REQUEST YOU TO PROVIDE THE FOLLOWING:
- Provide accurate and complete information regarding your illness, past medical history and other matters relating to your health.
- Bring your current prescription, reports and file with you at the time of visit so that they may be reviewed by your health care provider as required.
- Follow treatment instructions as advised and clear your all doubts if you do not understand anything related to medication.
- Collect all your reports / films etc. before leaving the hospital & if not ready then get a date for it when it can be collected from the respective department.
- Report any changes in your medical condition, including problems with your current treatment.
- Make informed decisions about your care and treatment and to accept responsibility for the decisions you make.
- Abide by CENTURY RAYON HOSPITAL TRUST rules and policies regarding Patient Conduct, NO SMOKING policy and Visitor policy.
- Respect other patients & their privacy, visitors and HOSPITAL STAFF. Be courteous with everyone.
- Pay your dues in time. Ask for estimated or interim bill if you have any doubts.
- Be on time for your appointment / admission. If necessary, reschedule or cancel appointments / admissions in advance so that this valuable time / bed may be given to another patient when possible.
- When your doctor orders your discharge from the hospital, prepare yourself to leave as soon as possible. Staffs are available to guide you about post discharge care.
I have read the rules and instructions mentioned above and hereby agree to abide them.
